(1) Whether a certain person (name furnished) has been employed as a consultant and has applied for employment in her department; if so, (a) in what capacity, (b) for what period and (c) at what rank; (2) whether any background checks were conducted on the said person to ascertain (a) his reasons for leaving his previous places of employment and (b) his state of health; if not, why not; if so, what did such checks disclose; (3) whether the said person has awarded any tenders or committed her department to expenditure in respect of (a) South African or (b) foreign service providers; if so, (i) what costs were incurred and (ii) what are the further relevant details?
