(1) Have any of the State Information Technology Agency’s client departments’ systems, at all spheres of government, suffered data (a) damage or (b) loss through hostile attacks on these systems during the past 12 months, if so, (i) what were the dates, (ii) which client departments and (iii) what were the extent of the (aa) damage or (bb) loss incurred; (2) what steps have been taken (a) to repair or retrieve the damaged or lost data and (b) to prevent recurrence of hack attacks?
