(1) Whether, pursuant to the tabling of the Gobodo Forensic Report into alleged administrative and financial irregularities at the Universal Service and Access Agency (USAASA), he has (a) studied the findings of the forensic report into the public entity in full and (b) has intervened decisively to restore stability and trust in the agency which is a key driver in the roll-out of digital terrestrial television; (2) (a) what are the full details of the steps he has taken against the USAASA Board for not exercising their fiduciary duties in terms of the Public Finance Management Act, Act 1 of 1999, (PFMA) and in terms of the Companies Act when they appointed the chief executive officer (CEO) against the advice of the Corporate Services Executive, (b) why did (i) he or (ii) his department not instruct the agency to terminate the contract of the CEO immediately after the Gobodo Forensic Report was tabled and before the probation period of the CEO had expired, (c) why did he not remove and charge the entire USAASA Board in terms of the Provisions of the PFMA for the irregular awarding of a 100% subsidy to Cell C where a former USAASA employee holds an executive position and a certain company (name furnished) for the irregular payments made to that company; (3) whether the more than R38 million paid into an unknown bank account for services rendered by the said company has been recovered; if not, what steps have been taken to recover the monies; (4) whether the amounts paid out to projects from public funds in the Universal Service Access Fund (USAF) have been properly reconciled and accounted for; if not, why not; if so, (a) what were the findings and (b) what are the relevant details of the steps that has he taken to revise the USAASA and USAF funding model to prevent corrupt activities; (5) whether (a) any legal costs have been incurred by his department in the disciplinary cases of USAASA employees who acted as whistleblowers; if not, what is the position in this regard; if so, what steps has he taken thus far to ensure the reinstatement of USAASA employees who were called to disciplinary hearings and dismissed when they acted in the best interests of the State to prevent financial losses through corrupt activities?
